Synchronous plasma cell neoplasm and B lymphoblastic leukemia/lymphoma at initial presentation: first report of an unusual association with a good outcome

A synchronous diagnosis of a plasma cell neoplasm (PCN) and non-plasma hematologic malignancy is very rare. We report what we believe the first instance PCN B lymphoblastic leukemia/lymphoma (B-ALL) diagnosed at initial presentation. The patient underwent laboratory evaluation for an underlying neoplasm, including immunology studies, bone marrow biopsy, flow cytometry immunophenotyping. Serum lambda free light chain serum IgG were elevated, with M-protein identified by protein electrophoresis immunofixation. clinical working was myeloma. Bone biopsy positive composite B-ALL. received treatment VDT-PACE chemotherapy followed autologous stem transplant maintenance therapy bortezomib/daratumumab in complete remission both diseases 3.5 years after diagnosis. This case not only adds to known repertoire neoplasms that can occur association PCN, but also demonstrates patients presenting this rare combination hematopoietic be effectively treated simultaneously excellent responses. Additional research warranted understand pathophysiology, identify potential prognostic factors, develop specific therapeutic plans these patients.
